We help families cope with changes in a relative's abilities, needs and behaviors, find solutions for care and other challenges associated with Alzheimer's disease, strokes and other dementia illnesses.
Depending on your needs, we offer any or all of the following:
- We conduct a comprehensive assessment to identify problem areas and needs for the person with memory loss and the caregiver.
- We create a written report of every visit and interaction with doctors and other health care professionals. This report, which answers your questions and concerns is shared with others, only as directed by you.
- We offer email, phone, and of course, in person consultations.
- We make referrals to doctors, financial and legal services, in-home agencies, community programs and residential care homes.
- We locate, schedule and monitor services, such as home care.
- We counsel caregivers and facilitate family meetings to help family members talk openly about their situations, what they can do and expect, and plan for the future.
- We assist families with transitions, from accepting help in the home to moving a relative to residential care.
